About The Position

As the Nurse Navigator, you will coordinate and manage the care of general surgical patients to ensure continuity of care delivered with compassion, excellence, and reliability by serving as a single point of contact for referring physicians, patients, and caregivers Serves in an expanded health care role and will provide resources, education and assistance with accessing clinical and supportive care services offered within the Munson Health Care system and surrounding communities You will facilitate patient appointments including those made for diagnostic services or clearances needed and be a resource for patients before and after surgery. Support quality improvement initiatives and adherence to safety standards.

Requirements

Licensed RN in the State of Michigan
Bachelor of Science degree in nursing preferred
High-Level understanding of surgical treatment modalities
Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella.
MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.
