About The Position

Incumbent will provide coordination and oversight for effective delivery of subspecialty care to patients in the assigned clinic. Incumbent serves as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient’s plan of care, and coordinates care with the physicians and members of the healthcare team.

Requirements

• Strong written, oral communication, as well as effective problem solving and negotiation skills
• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
• Must demonstrate customer service skills appropriate to the job
• Minimum 1 year experience working as an RN or 3 years MA/LVN experience in a clinical setting
• Experience coordinating care for patients with complicated medical diagnosis
• Excellent written and verbal communication skills in English
• Current California RN license
• Current BLS certification
• Computer familiarity using Word, Outlook and Excel, or similar database software
• All external candidates must have a Bachelor of Science in Nursing
• Ability to maintain a work pace appropriate to the workload
• Ability to establish and maintain effective working relationships across the Health System
• Ability to design and implement patient and family education programs

Nice To Haves

• Specialty nursing certification relating to patient population
• Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnosis, preferably urology
• Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnosis, preferably pulmonary and/or nephrology clinics
• Knowledge of University and medical center organizations, policies, procedures and forms
