About The Position

The Nurse Navigator is responsible for coordinating care and providing educational support to patients, ensuring a seamless care experience. This role involves collaboration with a multidisciplinary team to develop and implement care plans, facilitate referrals to specialists, and assist in discharge planning. The Nurse Navigator also plays a key role in enhancing clinical protocols and educational activities to improve patient care and community health outcomes.

Requirements

  • Bachelor's degree in nursing preferred.
  • Experience in community health, case management, or care coordination preferred.
  • Management and/or program development experience preferred.
  • Three years of acute care nursing experience preferred in area of specialization.
  • Ability to obtain and maintain a State of Nevada Registered Nurse license required at time of hire.
  • Current BLS by AHA standards required at time of hire.
  • Current ACLS by AHA standards required at time of hire.
  • Advanced cardiac certification (i.e., CCRN, CHFN, CACP, etc.) preferred within two years of hire.
  • Proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel, and Word.

Responsibilities

  • Coordinate care and provide educational support to patients and their families.
  • Assist department leadership in conducting programs that support clinical excellence.
  • Develop and implement up-to-date plans of care in collaboration with the multidisciplinary team.
  • Facilitate patient referrals to appropriate specialists for diagnosis or treatment.
  • Provide support and counseling to patients and their families as needed.
  • Assist in the development and performance of specialty service educational activities.
  • Work with marketing and outreach departments to educate referring physician offices on available services.
  • Participate in discharge planning and telehealth appointments.
