The Nurse Navigator serves as a clinical guide and advocate for oncology patients throughout the continuum of care, from initial screening and imaging through diagnosis, treatment, and follow-up. This position provides professional nursing support to coordinate multidisciplinary care, assist in overcoming healthcare system barriers, and enhance the patient and family experience.

Requirements

• Licensed Practical Nurse (LPN) required.
• Current State or Compact Nursing License required.
• Basic Life Support (BLS) certification required upon hire and maintained thereafter.
• Strong critical thinking and decision-making skills.
• Ability to work independently and in high-stress clinical settings.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Experience in oncology, case management, or patient navigation preferred.

Responsibilities

• Coordinates patient care across departments and providers from diagnosis through survivorship or end-of-life care.
• Consults with nursing staff and interdisciplinary teams to evaluate patient care, transitions, and discharge planning.
• Verifies that treatment plans align with patient needs and clinical standards.
• Provides education, emotional support, and advocacy for patients and families.
• Tracks and monitors patient progress, identifying and addressing barriers to care.
• Supports timely scheduling of diagnostic tests, treatments, and follow-up appointments.
• Participates in quality improvement initiatives related to patient navigation and satisfaction.
• Serves as a liaison between patients, providers, and community resources.
• Maintains detailed documentation and upholds patient confidentiality standards.
• Responds to questions and concerns from patients, families, and payers regarding care coordination.
