About The Position

The Connecticut Orthopedic Institute and Ayer Neuroscience Institute are highly dedicated to improving the lives of our patients. The Nurse Navigator serves as a patient advocate, liaison, and advisor/educator to guide patients throughout the continuum of care for their surgical experience beginning in the pre-operative phase up to a full year post-operatively to mitigate risks and promote positive outcomes. This position builds relationships with patients, families and the internal clinical team to improve patient outcomes, satisfaction and transitions through the continuum of care.

Requirements

  • BS in Nursing required. MS in nursing preferred.
  • CT RN license required. Certification in specialty area preferred.
  • Minimum 5 years clinical nursing experience preferred, particularly with orthopaedic or neurosurgical patients.
  • Competent with standard software (Word, Excel, Electronic Medical Record) and willing and able to learn additional software as needed to enhance navigation and coordination responsibilities
  • Knowledgeable regarding data interpretation and the tracking and evaluation of quality indicators
  • Demonstrates use of clinical skills and expertise in diagnosis, treatment, and psychosocial impact of disease.
  • Demonstrates excellent customer service skills including superior telephonic communications skills with the ability to comprehensively assess patient’s physical and psychosocial status as well as conduct complex medical history discussions over the telephone.
  • Ability to effectively and efficiently coordinate, implement and monitor the patient’s process of care, to communicate openly and empathetically with patients and their family members.
  • Ability to work independently as well as collaboratively with navigation team and multidisciplinary care team in all aspects of defining and implementing navigation program.
  • Ability to communicate effectively with diverse audiences including professional staff, patients, community, media, and other service agencies.
  • Knowledge of adult learning competencies and patient teaching skills.
  • Demonstrated management skills reflecting self-motivation, initiative and fiscal responsibility.
  • Tolerance of ambiguity and the flexibility to respond to changing system needs.
  • Demonstrates a commitment to own professional growth and competency.

Responsibilities

  • Serves as a member of the multidisciplinary care team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.
  • Navigates patients through the continuum of care, working with the multidisciplinary team to enhance patient and colleague experience.
  • Utilize electronic medical record reports and data management skills to coordinate patient flow.
  • Participate in community and colleague education initiatives
  • Assist in tracking and reporting of program performance measures, quality indicators, and contribute to the continuous improvement process.
