About The Position

The Nurse Navigator coordinates multidisciplinary care and supports movement disorders patient within the Chase Family Movement Disorders Center (CFMDC), a signature program of the Ayer Neuroscience Institute. This role provides guidance for patients through the continuum of care to eliminate barriers and enhance patient outcomes and satisfaction. The Nurse Navigator provides education, support, and logistical assistance, helping patients navigate the complexities of diagnosis, treatment, and recovery. This includes communicating the plan of care with patients, families, and providers across specialties to assure coordination throughout the continuum of care. Additionally, the Nurse Navigator will serve as a professional resource for data tracking and process improvement. Key priorities for this role include (1) supporting timely access to patient care, (2) facilitating effective communication and navigation across the care continuum, and (3) enhancing the overall patient/family experience and quality of care. The ideal candidate is an experienced registered nurse with a background in neurology or movement disorders, or a strong interest in this specialty. They bring excellent communication and organizational skills, proven ability to coordinate care across multidisciplinary teams, and a patient-centered approach grounded in compassion and dedication. Strong critical-thinking and problem-solving abilities are essential to supporting patients and improving quality of life.

Requirements

  • Licensed as a professional RN in the state of Connecticut
  • Minimum of 5 years nursing experience
  • EPIC experience
