About The Position

Join a world-class academic healthcare system, UChicago Medicine Crown Point , located in Crown Point, Indiana as a Breast Imaging Nurse Navigator (RN). UChicago Medicine is an integrated academic and community health system with multiple primary medical facilities within the surrounding suburbs of Chicago. UChicago Medicine, Crown Point is a two-story, 130,000-square-foot multispecialty care center and micro-hospital in Northwest Indiana, the academic health system's largest off-site facility and its first freestanding building in Indiana. The Breast Imaging Nurse Navigator (RN) provides individualized assistance to patients, families, and caregivers to help overcome health care system constraints and facilitate timely access to quality medical and psychosocial care from screening and pre-diagnosis of cancer through placement into appropriate treatment clinics. The Breast Imaging Nurse Navigator works closely with Physicians and staff in all specialty areas to coordinate the patients plan of care and communicates with all members of the team on behalf of the patient.

Requirements

  • Registered Nurse, in good standing, licensed in the state of Indiana.
  • Active BLS certification from the American Heart Association (AHA) .
  • Five years professional imaging, breast care, oncology nursing or oncology nurse navigation

Nice To Haves

  • National certification in Oncology and specialty practice area (e.g., CBCN, OCN, ONN-CG) preferred.

Responsibilities

  • Develope relationships with patients at first point of contact through diagnostic workup, placement into treatment clinics, and/or surveillance programs, as appropriate.
  • Asses patient, family and caregiver needs upon the initial encounter (first abnormal finding or initial diagnostic imaging services) including assessing barriers to care, performing distress screening as appropriate and making referrals to appropriate disciplines and documenting accordingly.
  • Works with the breast cancer-screening progam to identify patients with abnormal screening mammograms who require further workup.
  • Interfaces with outside referring physicians to communicate abnormal screen findings and offer services for further workup.
  • Assist with triaging patients for futher assessment.
  • Track pending pathology for all biopsies performed within Diagnostic Imaging and make referrals based on results.
  • Facilitates the entry of patients into the breast cancer program by reviewing breast imaging results and medical history indicative of breast cancer, triaging and referring newly diagnosed breast cancer cases to service for further care coordination.
  • Coordinate appointments for consults with support services within established service standards, including socialworkers, dieticians, genetic counselors, therapists, and others when needed.
  • Collaborate with DI Operations, access and other teams to obtain medical records, imaging/pathology, and insurance information to expedite initial consultation.
  • Collaborates with clinic staff, research nurses, and physicians to ensure appropriate Breast Imaging for potential and eligible clinical trial patients.
  • Apply knowledge of payer requirements and insurance processes and their impact on referrals for treatment and patient care decisions to set up appropriate treatment, as requested or needed.
  • Responds to voicemail and myChart messages within one business day and answers phone calls in a timely manner.
  • Other duties as assigned, including other leadership roles as required by the center.
