About The Position

The Community Practice Oncology Nurse Navigator (CPONN) is a professional Registered Nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help navigate the healthcare system. Using the nursing process, the CPONN provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The CPONN serves as an additional point of contact for patients, families, caregivers, and referring physicians to provide resources and assistance with accessing clinical and supportive care services offered within and outside of the assigned UCLA Hematology Oncology Community Practice Cancer Center.

Requirements

  • Knowledge of the Oncology patient population and practice operations.
  • Demonstrate strong interpersonal and organizational skills.
  • Ability to organize, prioritize, and follow through for multiple patient treatments.
  • Possession of a BSN or MSN degree.
  • Possession of current California RN license.
  • Possession of BLS certification from the American Red Cross or American Heart Association.
  • Possession of ONS/ONCC Chemotherapy/Immunotherapy certification.
  • Possession of OCN certification.
  • Minimum of five years of acute care/oncology inpatient and/or outpatient experience.
  • Experience in accurately screening and triaging acute patients.
  • Ability to recognize abnormal laboratory values and report to Physicians.
  • Ability to accept direction from leadership and act promptly.
  • Skill in listening perceptively and responding to patients and family/caregivers.
  • Ability to delegate tasks in a management style that promotes leadership and teamwork.
  • Ability to maintain confidentiality and work with sensitive information.
  • Working knowledge of national, regional, and community resources.
  • Knowledge of self-care strategies and resources.
  • Ability to collect and relay information with accuracy and diplomacy.
  • Excellent planning, directing, and coordinating skills.
  • Skill in operating a PC and PC-based software applications.

Nice To Haves

  • Possession of Academy of Oncology Nurse & Patient Navigators certification.
  • Working knowledge of the UCLA EHR (Epic/CareConnect).
  • Basic knowledge of insurance reimbursement systems.
  • Working knowledge of financial hardships and payer coverage.

Responsibilities

  • Provide individualized assistance to patients, families, and caregivers to navigate the healthcare system.

  • Serve as a point of contact for patients, families, caregivers, and referring physicians.
  • Educate patients and families about clinical and patient care issues.

  • Maintain cooperative working relationships with patients, peers, physicians, and administrators.

Benefits

  • Salary range: $70.41/hr - $102.30/hr
