About The Position

We are seeking a responsible, self-motivated individual to join our UNC Cancer team as a Nurse Navigator! This position qualifies for our UNC Hospitals RN Incentive Program, which includes $10,000 for a 3 year commitment. In addition, this position is also eligible for up to $10,000 in relocation assistance (based on location) and our employee referral program. In this role, you will be a vital member of the patient's interdisciplinary team guiding them through their cancer care journey. The Nurse Navigator role is a salaried position, working 40 hours per week, Monday-Friday. This position is mostly in-person with some opportunities for remote work (1 day per week), and offers many appealing benefits including health & retirement plans as well as generous PTO accruals. An experienced RN is needed to facilitate supportive care resourcing for oncology patients and their caregivers in the outpatient setting from diagnosis through treatment to survivorship or end of life. An oncology nurse navigator (ONN) is a professional Registered Nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers as a guide through the healthcare system. The navigator provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The navigator is responsible for identifying potential and realized challenges related to psychosocial barriers to care and facilitating referrals as appropriate to mitigate barriers and expedite the plan of care. The ONN assesses patients' current health-related social needs as well as concurrent health problems, determines the degree of compliance and problems complying with the prescribed treatment plan, and provides appropriate documentation. By analyzing data from patient interviews and chart reviews, the ONN helps to determine individualized needs including internal supportive care referrals through the Comprehensive Cancer Support Program as well as community-based resourcing in partnership with the Oncology Social Work team. The ONN will be instrumental in supporting and facilitating existing and new programming that prioritizes Cancer Survivorship Care as well as Caregiver Support, in addition to general support for patients and caregivers who self-refer to the Resource Center or are referred by their clinical teams. The ONN will partner extensively with the Oncology Patient Navigator (OPN) team to facilitate and guide the supportive care resourcing for patients and caregivers, and act as a clinical oversight and liaison for patients whose medical needs require input or intervention from their clinical care team.
