About The Position

A Day in the Life Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs. Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization. Support quality improvement initiatives and adherence to safety standards. The Nurse Navigator will coordinate and manage the care of OB/GYN/surgical patients to ensure continuity of care delivered with compassion, excellence, and reliability by serving as a single point of contact for referring physicians, patients, and caregivers. Navigator will provide resources, education and assistance with accessing clinical and supportive care services offered within the Munson Health Care system and surrounding communities. The Navigator will facilitate patient appointments including those made for diagnostic services or clearances needed and be a resource for patients before and after surgery/delivery. Reports to Director of Care Coordination.

Requirements

  • Must possess a current RN license to practice nursing in the State of Michigan.
  • Previous experience in Pre-Admission Testing, Case Management or clinical experience relevant to patient population being served is necessary.
  • Individual must be detail oriented, have computer application experience as well as excellent written and oral communication skills.
  • Position requires problem solving, decision making, and critical thinking skills.
  • BLS certification or obtain within 3 months of hire.
  • Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella.
  • MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.

Nice To Haves

  • Bachelor of Science degree in nursing preferred.
