About The Position

The Nurse Navigator serves as a crucial liaison between patients, physicians, and the care team, ensuring seamless coordination of care across various settings. This role focuses on patient education, advocacy, and support, aiming to enhance patient satisfaction and outcomes. The Nurse Navigator is involved in monitoring patient outcomes and participating in quality improvement initiatives, working collaboratively within a multidisciplinary team to positively impact patient experiences.

Requirements

• Associate of Science degree required.
• Active Registered Nurse license is mandatory.
• Three years of clinical experience in a hospital, managed care environment, or medical office.
• Knowledge of HEDIS and Star ratings, risk adjustment, and HCC coding preferred for BHQN.
• Proficient in computer applications and typing skills.
• Experience with Electronic Medical Record systems preferred.
• Bilingual skills are preferred.
• Preferred certification in area of expertise.
