About The Position

This job will assist the patient, family and care giver to overcome healthcare barriers (including but not limited to social, financial, or physical barriers.) Responsible for conducting on-going assessments of need and facilitate referrals throughout the nursing continuum of care. Using the nursing process and evidence-based knowledge, the Nurse Navigator provides education and resources to facilitate informed decision making (by the patient and/or family.)

Requirements

  • Graduate of an approved/accredited nursing program either by a governmental authority, nursing board, or by a licensing authority that has jurisdiction over approval/accreditation of nursing programs (Texas BON 217.1)
  • 3 years of clinical nursing experience
  • Specialty areas may require 3 years of specialized nursing experience.
  • (RN) REGISTERED NURSE Licensure as Registered Nurse of Board of Nurse Examiners for State of Texas Upon Hire
  • (BLS) BASIC LIFE SUPPORT - current Basic Life Support (BLS) certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire

Responsibilities

  • Assesses for any barriers to care through the clinical continuum to address patient, care giver or family needs.
  • Serves as point of contact for patient, care giver, and clinical care team.
  • Serves as patient advocate to assist patient and care giver through the healthcare system.
  • Provides education, resources and referrals to internal and external resources based off of assessment need.
  • Facilitates shared decision making, ensuring the patient, family and care giver are fully informed of treatment intent, treatment and outcomes.
  • Promotes care planning by encouraging patients to convey their wishes and goals. Works with patients to formulate questions to discuss with their healthcare team.
  • Facilitates clear and concise communication among providers, departments and patient.
  • Coordinates multidisciplinary planning and facilitate care conferences, as necessary.
  • Facilitates education of patient and staff by developing appropriate education programs, resources and tools to address needs.
  • Participates in outreach projects to help close patient care gaps.
  • Performs other duties as assigned.
