About The Position

The Nurse Navigator II is an integral part of the Oncology Multidisciplinary Team.The NN II supports, educates, identifies barriers to care and assists patients and families adjust to a new cancer diagnosis. This role helps patients navigate the complexity of the healthcare system and contributes to improved patient outcomes and patient and provider satisfaction.

Requirements

• 3-5 Years required
• Master's Degree Required
• New Jersey Registered Nurse ☒
• Master’s Degree in Health Care related Field ( see above) ☒

Nice To Haves

• Oncology Nursing experience and program development background a plus

Responsibilities

• Connects with newly diagnosed patients prior to first apt and at first appointment with oncology provider
• Facilitates the Multidisciplinary Care at MDA Cancer Center at Cooper University Health Care
• Supports, educates and removes barriers to ensure timely specialized care
• Assesses patients for Clinic Research Trials
• Coordinates Disease Specific Tumor Board
• Complies with all system and departmental policies and procedures
• Maintains active and current professional credentials
