About The Position

The Nurse Navigator I plays a foundational role in shaping the client experience. This role focuses on subacute client needs, supporting the intake process, building co-created care plans remotely, and serving as the primary clinical support for a dedicated caseload. You’ll triage inbound clinical questions from nurse contractors, maintain clear communication with clients and families, and escalate higher-acuity needs to the Nurse Navigator II or Nursing Manager. This position is ideal for RNs who enjoy virtual care coordination, clinical problem-solving, and creating trust through strong, empathetic communication. Note: While this role does not require routine field travel, travel to the office or as coverage on client sites may be required based on business needs.

Requirements

  • Registered Nurse (AZ), unencumbered.
  • 4+ years of direct client care experience, including at least 1 year in acute care within the last 5 years.
  • Current AHA BLS and/or ACLS.
  • ASN or higher from an accredited nursing program.
  • Comfortable providing primarily virtual client support.
  • Excellent phone and digital communication skills.
  • Strong critical thinking and clinical judgment.
  • Tech-fluent: Google Suite, EHR platforms, telehealth tools, and quick to adopt new systems.
  • Thrives in a fast-paced, autonomous, startup environment.
  • Completion of 20+ hours of in-person training at Navi HQ.
  • Navi contractors with 6+ months experience are highly encouraged to apply.

Responsibilities

  • Conduct virtual intake assessments and build co-created care plans for subacute clients.
  • Manage the ongoing clinical needs for an assigned caseload.
  • Triage inbound questions from nurse contractors; provide clinical support and guidance.
  • Appropriately escalate client concerns to Navigator II or the Nursing Manager.
  • Communicate proactively with clients, families, and clinical nurses regarding updates, changes, and next steps.
  • Maintain high-quality, timely documentation and charting.
  • Support care coordination between clients, nurse contractors, and internal teams.
  • Serve as a brand ambassador for the Navi experience — warmth, clarity, and high-touch service.
  • Participate in internal meetings, QA initiatives, and process improvements.
