About The Position

Be a part of a world-class academic healthcare system, UChicago Medicine, as a Nurse Navigator at our DCAM - Hyde Park clinic. Here, you will work alongside our Digestive Disease - Inflammatory Bowel Disease (IBD) team. This position is a 100% onsite opportunity and requires you to be based in the greater Chicagoland area with local travel. In this role, you will work as the Nurse Navigator for inflammatory bowel disease (IBD). Our GI specialists offer exceptional care based in advanced, research-driven medicine for both common and rare GI conditions, and we are one of the nation’s top-ranked hospitals for gastroenterology and GI surgery. Under the direction of a faculty sponsor and in collaboration with the program support team, you will coordinate the care of patients through the disease process, acting as a clinical liaison across medical services and clinical departments. You will assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs, and evaluate and manage patient symptoms before, during, and after treatment.

Requirements

• Must be an actively licensed Registered Nurse in the state of Illinois
• Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients.
• Must have general computer skills and be adaptable to changing working situations and work assignments.
• Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.

• Certified Nurse in Specialty area or in process.

Responsibilities

• Provides care coordination & patient education to patients in IBD service line.
• Supports discharge planning for the IBD service line.
• Facilitates resolution to patient care gaps and monitors follow up/completion by both proactively reaching out to patients and addressing direct phone calls and messages from patients.
• Acts as liaison for patients, families and their multidisciplinary clinical team.
• Educates/reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan.
• Clinical documentation review to improve LOS.
• Develops and/or coordinates development of patient education materials, clinical pathways, and quality studies.
• Serves as primary point of contact for patients discharged from the hospital to address questions and patient care needs to prevent readmission or an emergency room visit.
• May be responsible for direct patient care
• Performs other duties assigned.
