About The Position

The Nurse Manager ensures access to the highest quality care for all ChesPenn patients by partnering with other leadership staff in planning for clinical support staffing, developing policies and procedures for nursing and other medical support roles, creating staff development policies, procedures, and tools, supervision of direct reports, and modeling exemplary nursing care.

Requirements

  • Bachelor’s degree in nursing or an Associate Degree in Nursing with relevant experience
  • Current Registered Nurse (RN) license in the state of Pennsylvania
  • Experience in acute care, community health, or outpatient settings is highly desirable
  • Proficiency in EMR systems for documentation and patient management
  • Strong communication skills with the ability to educate diverse populations
  • BLS certification required; additional certifications (e.g., ACLS, PALS) are a plus
  • Moderate physical effort in carrying charts, forms, supplies and equipment.
  • Primary work environment is a multi-site, multi-cultural ambulatory care setting. There is routine exposure to infectious disease and bloodborne pathogens

Responsibilities

  • Supervise and Manage nursing staff
  • Partner with other senior staff in developing and implementing policies, procedures, and training systems for nursing and medical assistant staff
  • Participate in performance evaluation of clinical support staff
  • Analyze and report on clinical staff efficiency and effectiveness
  • Ensure appropriate documentation in all clinical support activities
  • Provide professional nursing services including triage of patients
  • Prioritize patients according to intensity, need, and required follow-up and develop complete plans of care to address the needs of patients.
  • Perform follow-up calls for patients recently discharged from acute hospitalizations and who are considered high risk for readmission
  • Update patient medical records after each contact
  • Perform medication management, including reconciling discharge medications with ongoing medication regimens and developing a patient-directed-self-management strategy for compliance
  • Work closely with providers and health care team to ensure the patient an appropriate plan of treatment and a safe transition to home
  • Promote patient self-management and empower patients/families to achieve maximum levels of wellness and independence. Interacts professionally with patient/family and involves patient/family in the formation of a plan of care
  • Manage chronic and acute health problems
  • Counsel and educate patients on management of their illness
  • Support medical providers as needed
  • Offer education and guidance for navigating complex medical decisions
  • Maintain EMR databases on care managed population
  • Maintains accurate and timely documentation in the EMR
  • Encourage and work collaboratively in team-based care
  • Other duties as assigned by the Chief Executive Officer or the Director of Clinical Support
