About The Position

Abernathy Home Care is seeking an experienced Registered Nurse (RN) to join our leadership team as a Nurse Manager. We are a private home care agency providing services through local ASAP programs, VA contracts, and private-pay clients. This is a full-time, salaried, field-based leadership role focused on care plan oversight, client assessments, staff supervision, and compliance across multiple service lines. This role is ideal for an RN with home care or community-based experience who enjoys field leadership, care coordination, and quality oversight, and is seeking predictable hours and strong work-life balance.

Requirements

  • Active Massachusetts RN license (required)
  • Home care, community health, or field-based nursing experience required
  • 3–5 years of nursing experience with care coordination or case management
  • 1–2 years in a supervisory, nurse manager, or leadership role
  • Experience working with ASAP programs, VA services, or private-pay clients
  • Familiarity with care plans, assessments, and payer documentation standards
  • Strong assessment, organizational, and communication skills
  • Comfortable working independently in the field
  • General computer and technology proficiency for documentation and communication
  • Valid driver’s license and reliable transportation

  • Bilingual Spanish preferred, not required

Responsibilities

  • Develop, implement, and review individualized Plans of Care
  • Conduct in-person client assessments and determine service eligibility
  • Provide field-based supervision and guidance to clinical and non-clinical staff
  • Serve as a leadership resource for caregivers and field teams
  • Maintain a consistent full-time field presence
  • Ensure compliance with ASAP guidelines, VA requirements, and private-pay service standards
  • Support audit readiness and quality reviews through accurate documentation
  • Assist with preparation and follow-up for ASAP, VA, and payer audits
  • Collaborate with leadership to improve service delivery, quality, and client satisfaction
  • Participate in quality improvement and compliance initiatives
