About The Position

Coordinates with Center Manager in hiring of nursing staff and Clinical Support Coordinator. Maintains responsibility for training and supervision to conduct Career Development Review(s) for assigned staff on a timely manner. Develops competency and evaluates staff on competency of nursing care and performance according to establish standards and expectations set during orientation and annually. Maintains appropriate staffing levels according to provider schedules and nursing care needs making adjustments when necessary within budgetary guidelines. Performs payroll activities in a timely manner for assigned staff. Coordinates work flow among members of the care team including but not limited to the Care Management Nurse(s), Integrated Behavioral staff and Community Health Worker(s) by use of team huddles, quality reporting and report cards to ensure optimal patient care in the Patient Centered Medical Home. Maintains patient flow in conjunction with the Clinical Support Coordinator. Provides triage services per GLBHC protocol. Assists with tracking and follow up of care managed patients, works as an active member of the nursing team, assisting with patients as needed to expedite patient care. Listens to patient concerns and/or complaints and follows up as appropriate. Monitors filing, tracking, patient notification of test results, etc. to ensure timeliness of these activities and proper turnaround time frames. Communicates effectively with providers, staff, patients and supervisors. Keeps staff well informed of changes and documents as appropriate. Initiates, implements and monitors quality improvement activities to enhance provision of care. Ensures that quality control checks on equipment and supplies used for patient care are performed per standard and protocol and acts appropriately on variances. Maintains employee health records and ensures all employees at site are up to date on immunizations and screenings per the Employee Health protocol. Collaborates with the Clinical Support Coordinator to maintain the Vaccines for Children (VFC) and private stock inventory. Responsible for stock medical inventory and ordering of stock medications. MARGINAL JOB DUTIES Keeps updated on laws and regulations, accreditation requirements, and patient care standards as they relate to medical practice and nursing care. Participates in committees and work groups as needed. Performs other duties as assigned.

Requirements

  • Associates Degree in Nursing required.
  • Graduate of an accredited school of nursing with BSN preferred. In lieu of BSN, an RN with 3 years of Primary Care experience accepted.
  • Registered Nurse licensed by the State of Michigan.
  • Maintain a Basic Life Support Certification (BLS).
  • 1-2 years previous supervisory/management experience.
  • Knowledge of local health/social system.
  • Problem identification and resolution, working toward mutually beneficial situations.
  • Self-motivated.
  • Decision maker.
  • Able to function with minimal daily supervision.
  • Organized and detail oriented.
  • Ability to communicate effectively with a diverse population, including patients, providers and referral sources.
  • Ability to work and communicate effectively with a diverse population.
  • Ability to promote confidence, cooperation and good working relationships between management and staff which are fair and consistent.
  • Must be able to sit, stand, and or walk for an entire workday.
  • Must be able to lift, carry, push, pull, and or twist while holding up to 25 lbs. occasionally.
  • Full-time; Monday through Friday.
