About The Position

JOB SUMMARY The Nurse Manager of Memory Support/Namaste oversees daily nursing care for residents in the Memory Support and Namaste areas, focusing on their physical and mental well-being. The Manager works as a practicing LPN or RN, supervises other clinical staff, and manages emergency responses when needed. This position collaborates with Cognitive Services, Social Work, and other departments to ensure residents’ needs are met across medical and psychosocial domains.

Requirements

  • Licensure: Must possess a current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the Commonwealth of Virginia.
  • Experience: Two (2) years of nursing experience in long-term care or assisted living setting, required
  • Certification: Must have valid CPR certification or obtain it after employment.
  • Must obtain Certified Dementia Practitioner (CDP) certification within 6 months of employment.
  • Comprehensive knowledge of nursing procedures, theory, and practice, with a focus on current trends in gerontological nursing.
  • Knowledge of different types of dementia and programs that delay the progressive cognitive deterioration of people with dementia.
  • Exceptional interpersonal and communication skills with residents, families, and interdisciplinary teams.
  • Ability to work as a team or independently; self-motivated and goal-oriented with shared accountability.
  • Excellent organizational and administrative skills with the ability to handle multiple priorities in a fast-paced environment.
  • Ability to maintain professional working relationships and work compassionately with employees, residents, family members, and the public.
  • Ability to take initiative to solve problems and exercise sound professional judgment in emergency situations.
  • Strong supervisory skills with the ability to motivate, train, and evaluate staff; skilled in resolving workplace conflicts.
  • Proficiency with Google Suite, Microsoft Word, and Microsoft Excel; experience with electronic medical records (EMR) systems.

Responsibilities

  • Oversee and manage the performance of nursing staff, including assigning work, training, and conducting performance evaluations.
  • Develop and manage staff schedules; ensure daily staffing levels are maintained to provide safe resident care and support finding necessary coverage to maintain safe staffing levels.
  • Participate in interviewing and hiring clinical staff.
  • Ensure staff maintain professionalism and positive interactions with residents, families, and other employees.
  • Continuously evaluate work processes for improvement and support staff development through training and advancement opportunities.
  • Conduct daily rounds to monitor staff and residents, ensuring effective communication and resident well-being.
  • Coordinate resident medical appointments and referrals to ancillary services (therapy, psychiatric evaluations, etc.) and ensure paperwork is prepared.
  • Observe residents for changes in health or behavior and report concerns to appropriate parties.
  • Conduct audits of medication administration and nursing practices; provide retraining as needed.
  • Manage prescription orders and dispose of medications following protocols.
  • Respond to resident needs, including emergencies, medication administration, and support with daily activities when necessary.
  • Communicate with family members as appropriate.
  • Maintain compliance with Department of Social Services (DSS) regulatory guidelines for both Memory Support and Namaste.
  • Complete assessments, care plans, and documentation in compliance with DSS regulations.
  • Collaborate with Cognitive Services and the Social Worker to complete the Uniform Assessment Instrument (UAI) and update Individualized Service Plans (ISP).
  • Drives critical conversations with families regarding appropriate levels of care and transitions of care.
  • Conduct chart audits for regulatory compliance and maintain accurate, up-to-date documentation on residents’ care plans and activities.
  • Prepare and participate in regulatory surveys.
  • Participate in the On-Call Manager rotation to handle after-hour emergencies and urgent needs, including resident and staffing issues.
  • Maintain unit supplies, equipment, and records for ordering needs.
  • Take responsibility for professional development by attending required training programs.
  • Perform other duties as assigned.
