Reporting to the Director of Nursing and Site Medical Director, the Nurse Manager in Training Supervisor position is intended for Nurse Managers employed by Outer Cape Health Services, Inc. stepping down from the role or transitioning to a new role with Outer Cape Health Services, Inc. This position promotes the seamless transition between outgoing and incoming Nurse Managers to best serve our patients and employees through minimal disruption in management changes or vacancies. The Nurse Manager in Training Supervisor will overlap in the Management role with the Nurse Manager in Training for a period of two weeks before transitioning to part-time hours. The Nurse Manager in Training Supervisor performs all clinic-based tasks appropriate to Team Based Care Nursing and serves as the primary resource to the Nurse Manager in Training for a period of nine (9) months; creating an environment to support patient-centered, team-based care. As a member OCHS clinical staff, the incumbent is responsible for implementing and enforcing established OCHS policies and procedures and for fulfilling administrative tasks, as assigned.

Requirements

  • Bachelor of Science in Nursing is required from an accredited school of nursing.
  • Current RN license from the Massachusetts State Board of Registration.
  • BLS certification required.
  • Minimum of four years of nursing supervisory experience with Outer Cape Health Services required.
  • Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
  • Excellent written and verbal communication skills – bi-lingual helpful
  • Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.
  • Excellent judgment and ability to solve problems in a timely manner.
  • Ability to adhere to strict confidentiality standards.
  • Demonstrated ability to work effectively both as a team member and independently.
  • Demonstrated ability to work in a culturally diverse and inclusive environment.
  • This is a physical job by nature, conducted in a clinical environment with access to an office for administrative tasks. Requires being able to stand for long periods of time as well as the ability to use computers and a phone.

Responsibilities

  • Provide guidance and serve as a resource on day-to-day nursing operations and training for the Nurse Manager in Training contributing to quality and efficient patient outcomes
  • Provide guidance and serve as a resource to the Nurse Manager in Training on collaboration with Lead MA to manage staffing levels by ensuring nurse and MA staff schedules are developed, communicated and fulfilled
  • Provide guidance and serve as a resource to the Nurse Manager in Training on collaboration and ensure that new RNs, LPNs and MAs receive training and site orientation, forwarding completed documentation to Human Resources for personnel file
  • Provide guidance and serve as a resource to the Nurse Manager in Training on collaboration on coordination of CLIA, OSHA and other clinical employee training as needed
  • Provide guidance and serve as a resource to the Nurse Manager in Training on collaboration with provider leadership, Director of Nursing, Site Medical Director, and other members of the team to resolve conflicts
  • Serve as mentor and resource to the Nurse Manager in Training and other members of the health care team to optimize overall performance and maximize patient satisfaction
  • Provide guidance and serve as a resource to the Nurse Manager in Training on review and monitoring Patient Experience ratings and comments, patient problems and concerns in collaboration with providers and Site Medical Director
  • Provide guidance and serve as a resource to the Nurse Manager in Training on collaboration with others to develop/revise effective workflows and procedures and communicate, monitor, improve and maintain these workflows as necessary
  • Provide guidance and serve as a resource to the Nurse Manager in Training on promotion and facilitation of on-site process improvement working with frontline staff
  • Provide guidance and serve as a resource to the Nurse Manager in Training on recommending and implementing opportunities for revenue enhancements and expense reductions to optimize OCHS’s revenue stream
  • Provide guidance and serve as a resource to the Nurse Manager in Training on scheduling and leading regular RN, LPN and MA staff meetings including: Meeting cadence Agenda setting Facilitation of efficient meetings
  • Provide guidance and serve as a resource to the Nurse Manager in Training on the ordering of all clinical supplies, immunizations and vaccines to ensure adequate supply for clinical care; maintain required documentation and minimize waste
  • Provide guidance and serve as a resource to the Nurse Manager in Training on serving as Vaccine Coordinator for site, assist Nurse Manager in Training in enrollment in and completion of required education.
  • Provide guidance and serve as a resource to the Nurse Manager in Training on RN, LPN and MA current immunization schedules, available immunizations and ensuring compliance.
  • Provide guidance and serve as a resource to the Nurse Manager in Training on development of performance improvement plans and participation in formal disciplinary processes for RN, LPN and MA staff
  • Provide guidance and serve as a resource to the Nurse Manager in Training on data collection, reporting, and adherence to quality standards
  • Provide guidance and serve as a resource to the Nurse Manager in Training on consistent and fair discipline to staff and hold staff accountable for complying with OCHS policies and procedures
  • Provide guidance and serve as a resource to the Nurse Manager in Training on annual competency assessments for RNs and Mas
  • Consistently model, reinforce, and support tenets of patient-centered, team-based care
  • Conduct nurse visits including blood pressure checks, wound care, injections, allergy shots, INR checks per standing orders, vaccine administration per standing orders or provider direction, and other visits per protocol.
  • Perform triage using clinical protocols, assess patient needs, and coordinate same-day care when necessary.
  • Exhibit high level of clinical acumen and decision making to assist in the overall care of patients’ acute and chronic needs as part of a highly functioning clinical team.
  • Provide medication education and reinforce treatment plans for chronic disease management (e.g., diabetes, hypertension, asthma).
  • Perform medication refills per standing protocols.
  • Assist with post-hospital discharge follow-up and care transitions.
  • Conduct routine patient examinations and assessments according to scope of practice based on licensure and OCHS protocols.
  • Provide the best patient experience possible; address patient’s concern and questions, explain procedures, testing, and diagnosis thoroughly.
  • Educate patient in the proper use of medical equipment, prescribed regimens and other nursing care regimens.
  • Refer patient to outside sources, programs, or support groups, for additional education or referrals.
  • Other related duties and projects, as required or assigned in order to ensure the needs of the Health Center and its patients are met.
  • Collaborate with providers and care team members to support panel management and ensure timely follow-up on labs, referrals, and preventive services.
  • Participate in pre-visit planning with a focus on leveraging standing orders and written protocols to ensure patients are staying up to date on care gap and chronic disease management.
  • Review dashboards and care gap reports to outreach to patients and schedule necessary visits.
  • Participate in team-based huddles, case conferences, and interdisciplinary care planning.
  • Work collaboratively within the care team to ensure seamless patient experience.
  • Contribute to quality improvement projects and help implement population health strategies.
  • Support coordination of specialty care, social services, and community-based resources.
  • Maintain patient records with accurate and timely documentation in the Electronic Medical Record (EMR) in accordance with OCHS policies.
  • Ensure compliance with infection control standards, medication safety, and clinical protocols.
  • Maintain patient privacy and confidentiality of information at all times in accordance with OCHS policies and HIPAA.
  • The Nurse Manager in Training Supervisor will ensure the Nurse Manager in Training is competent in all aspects below by the end of the nine (9) month period: Team Based and Direct Patient Care & Clinical Support
  • Designated clinical competencies will be assessed upon hire and annually. These competencies may include Patient Triage, INR Monitoring, Ear lavage, AED, maintaining sterile field, wound care, Pain Assessment, and other requirements depending upon introduction of new procedures or equipment.
  • Collaborate with health care providers, nurses, medical assistants, and other team members to provide quality and effective patient care
  • Administer medications, start and maintain IV’s, provide patient teaching, and perform nursing procedures as ordered
