Responsible for ensuring the coordination of home health, hospice, and infusion services for the hospitalized patient, clinic/outpatient and/or patient in an alternative setting. Ensures delivery of quality patient care and compliance with clinical, administrative policies and procedures as well as regulatory standards. Responsible to interface with, provide Referral Source education, and address concerns.

Requirements

  • BSN  is required.
  • Current RN licensure in the state of NC or compact license from participating state is required.
  • BLS (may be renewed during orientation)
  • Valid NC Driver’s license
  • 5 years of clinical practice with at least 2 of those years being in supervisory role required.
  • Experience in home-based care environments or care coordination required.
  • Working knowledge of Medicare and Medicaid regulations including JCAHO
  • Excellent communication & organizational skills required.

  • MSN is preferred
  • Computer skills including Word and Excel preferred.

Responsibilities

  • Oversee daily clinical operations of BD team. Manage, develop and coordinate clinical operational activities on a 24-hour basis.
  • Assist the Director of Business Development in the creation of strategic and operational plans and department annual budget.
  • Collaborate with the Director of Business Development to determine fiscal requirements of the department, assist with budget preparation, complete biweekly and monthly payroll, assess appropriateness of resource utilization. Develop action plans in conjunction with the Director of Business Development to ensure budget and resource efficiency.
  • Participate in various personnel actions including hiring, performance reviews, disciplinary actions, terminations and other related activities.
  • Evaluate clinical practice and review clinical documentation to ensure optimization of best practice care delivery and clinical outcomes.
  • Maintain current knowledge of Medicare and Medicaid regulations along with all state and national governing body regulations.
  • Maintain current knowledge of the comprehensive services, products and supplies available through DHCH.
  • Remain current on reimbursement issues and their impact on the delivery of DHCH program services.
  • Ensure appropriate staffing coverage at all 3 DUHS hospitals.
  • Assess and coach staff level of practice. Develop training programs or tools to foster continued clinical development.
  • Develop / Revise departmental policies to improve efficiency and support best practice.
  • Participate on interdepartmental committees to foster teamwork and improve the delivery of quality patient care.
  • Participate with Referral sources to identify patient population needs, identify referral process improvements and staff educational opportunities.
  • Other duties as assigned.
