About The Position

Flexible schedule! As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services to adult clients enrolled in the Assertive Community Treatment (ACT) program. ACT is a program that provides services through less traditional service delivery methods. This is very much a "meet the client in their community" approach. You will support the RN and the Psychiatric Nurse Practitioner on the ACT team and be responsible for meeting with clients and aiding in their medication administration and additional needs. You will report to the Director of the ACT Program, a Licensed Mental Health Professional and work onsite and in the community. Review lab and test results, reporting critical values to RN and providers Medication management including ordering medications and obtaining prior authorizations Dispense medications; picking up medications, creating medication boxes, overseeing client medication administration Communicate with Director and ACT team regarding observation in client’s health/behavior Provide client and family education regarding medications and treatments Meet with clients and families within their homes and communities within Saratoga, Warren and Washington counties. Collaborate with the ACT Team during daily team meetings regarding concerns and behaviors Nursing documentation using the electronic health record Support all admissions and discharges of clients and their medication needs Work in collaboration with service providers and pharmacies to support client needs. Other duties as assigned within scope of practice and based on agency needs

Requirements

  • LPN Licensed in New York State
  • CPR Certification required.
  • 1 to 2 years of experience
  • NYS License required as travel is required.
