About The Position

Lifeskills Behavioral Health offers a residential treatment program in Deerfield Beach, FL, for clients suffering from mental health and substance use disorders, aiming for sustained recovery. The program provides a secure and nurturing atmosphere in fully furnished townhomes, professionally staffed 24-hours a day, allowing residents to practice new coping skills within the community. Lifeskills focuses on underlying disorders using a comprehensive whole-person approach for client-centered care, treating adults aged 18 and older, particularly those resistant to medication, exhibiting risk-taking behavior, or with multiple treatment episodes. The facility provides 24/7 nursing support in a structured therapeutic environment. The team is dynamic, dedicated, and passionate about delivering evidence-based, personalized clinical care. The Licensed Practical Nurse (LPN) interacts with clients and team members, assisting direct care staff with functional needs and collaborating to maintain a safe and therapeutic environment. The LPN performs medical care tasks including taking physician orders, preparing and administering medications, assessing and documenting client health, and coordinating health/medical services according to physician orders.

Requirements

  • Must hold a current LPN licensure within the state of Florida.
  • Have an associate or bachelor’s degree in nursing.
  • Active First Aid/CPR/AED certification.
  • Clear TB skin test or chest X-ray.

Responsibilities

  • Provides a comprehensive nursing assessment identifying client’ s medical needs, capabilities, and limitations.
  • Records and transcribes physicians written and verbal orders.
  • Carries out or delegates STAT orders immediately.
  • Communicates observations and recommendations for clients’ plan of care, providing input into the treatment team process.
  • Identifies abnormal diagnostic data and brings to the attending physician’s attention.
  • Documents the administration of medication (including PRN) in according to established policies and procedures.
  • Coordinates all pharmacy and laboratory orders, delivery, and refills with the appropriate vendors.
  • Responsible for the security of all controlled medications kept in the facility and monitors the administration and accurate recording of use.
  • Administers medications according to physician orders and medication administration record.
  • Administers injections, as needed/ordered.
  • Provides, structures, and maintains a therapeutic environment in collaboration with other staff.
  • Documents observations in nursing assessment and weekly notes in electronic charting system, Best Notes, in accordance with facility policy in a timely manner.
  • Documentation reflects plan of care outlined in the Master Treatment Plan.
  • Documentation reflects ongoing assessment of resident behaviors and feelings as expressed and are addressed appropriately within the program or with referral.
  • Prepares clients for discharge through individualized counseling and instruction prior to discharge in a clear and concise manner; providing written instructions if needed.
  • Provides education to clients and families about medications, side effects, contraindications, and physical symptoms through individual counseling and group counseling.
  • Provides review of clinical records for accuracy and quality documentation.
  • Assists in the coordination of admissions, transfers, and discharges to provide a smooth and efficient flow of communication.
  • Coordinates facility infection control program ensuring staff and clients meet policy and procedure requirements.
  • Can be relied upon to be at work as scheduled and is rarely absent from work, notifying the supervisor if absent.
  • Demonstrates an ability to adapt to changes in the facility function, management styles, and facility routines.
  • Reads, understands, and adheres to all company policy statements on ethics, conduct, and conflict of interests.
  • Adheres to facility policies, procedures, rules, and regulations, implementing regularly.
  • Maintains positive attitude and acts as a team player with others on the treatment team.
  • Other duties as assigned.
