Together, let's drive excellence in healthcare. SUMMARY OF JOB Delivers quality hospice and palliative nursing care to patients in the community, nursing home, or inpatient hospice setting. Develops, implements, and coordinates the interdisciplinary plan of care for each patient and family. Participates in professional development activities that further personal and program goals related to end of life care. Requirements

  • Graduate from an accredited school of nursing.
  • Minimum 1 year med/surg, nursing home, or rehab or 2 years’ experience as a professional nurse.
  • Able to meet weekend and holiday requirements.
  • Current licensure in State of practice
  • Valid Driver’s License
  • Current CPR Certification

Nice To Haves

  • BSN preferred
  • 1 year home care or hospice experience preferred.
