Job Overview The Nurse Home Visitor provides in-home support and education to first-time mothers, guiding them through pregnancy and the first two years of their child’s life. They focus on improving maternal and child health outcomes, supporting family economic self-sufficiency, and connecting clients to community resources as needed. Through building long-term therapeutic relationships, conducting nursing assessments, and collaborating with clients to set goals, they deliver care in alignment with the Nurse-Family Partnership model. As an integrated health organization Acenda values the use of evidence-based practice or EBP's. Program Info The Nurse-Family Partnership Program is a free service that pairs a first-time mom by 28 weeks gestation with a personal registered nurse to ensure the best possible pregnancy outcome, the best health and development of the baby, and to help link mom to community resources.

Requirements

  • Bachelors degree in Nursing (BSN) required
  • Active NJ Registered Nurse license (in good standing)
  • Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record
  • Professionalism and a commitment to excellence in care

Responsibilities

  • Travel to clients' homes and provide home visits to first-time mothers providing education and support through pregnancy and up until the child's 2nd birthday.
  • Work to improve pregnancy outcomes, child health and development outcomes, and the economic self-sufficiency of families.
  • Build long-term therapeutic relationships with clients and their families.
  • Provide community resources as needed per nursing assessment.
  • Adheres to the nursing process and the Nurse-Family Partnership model of home visitation.
  • Attends and participates in case conferences and team meetings.
  • Collaborates with client and family to develop short- and long-term goals.
