About The Position

The Telephonic Nurse Disease Management II at Elevance Health is responsible for delivering complex patient education and disease management interventions, focusing on health improvement programs for chronic diseases. This role involves conducting assessments, coordinating health coaching plans, and collaborating with healthcare providers to ensure effective care for members. The position is primarily virtual, requiring candidates to reside within 50 miles of an Elevance Health Pulse Point, and offers a structured work schedule from Monday to Friday.

Requirements

  • Current unrestricted RN license in applicable state(s) required.
  • Requires AS in nursing and a minimum of 3 years of condition-specific clinical or home health/discharge planning experience.

Nice To Haves

  • BS in nursing preferred.
  • Prior case management experience is preferred.
  • Prior disease management experience is preferred.
  • Experience with Coronary Artery Disease and COPD is a plus.

Responsibilities

  • Conduct behavioral or clinical assessments to identify individual member knowledge, skills, and behavioral needs.
  • Identify and coordinate specific health coaching plan needs to address objectives and goals identified during assessments.
  • Interface with providers and other health professionals to coordinate health coaching plans for the member.
  • Implement and coordinate coaching and care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs.
  • Use motivational interviewing to facilitate health behavior change.
  • Monitor and evaluate the effectiveness of interventions and health coaching plans and modify them as necessary.
  • Direct members to facilities, community agencies, and appropriate providers/networks.
  • Refer members to catastrophic case management as appropriate.
  • Serve as a resource to lower-leveled DM associates.
