The Outpatient Nurse Coordinator supports the Division of Infectious Diseases by coordinating and managing care for patients receiving outpatient treatment for infectious conditions, including complex antibiotic therapies. This role ensures safe transitions of care, continuity between inpatient and outpatient settings, and high-quality patient education while working collaboratively with the interdisciplinary care team to support Cooper University Health Care’s mission of delivering coordinated, patient-centered care. Based at the Division of Infectious Disease Office at 101 Haddon Ave, the RN work collaboratively with the physicians, advanced practice providers, and outpatient staff in the Division of Infectious Disease to support the outpatient ID clinics at 3 Cooper Plaza in suites 104 and 513, at MD Anderson at 2 Cooper Plaza, at 2339 Rt 70 East, Cherry Hill, and at 400 NJ-38, Moorestown. Function as a clinical care liaison between the Complex Outpatient Antibiotic Team and outpatient providers and staff for hospitalized patients who are discharged on treatment for infections. Demonstrate clinical knowledge of antibiotics and their indications and care of indwelling lines and home antibiotic infusions to able to educate patients about their conditions and treatment plans. Use clinical skills and judgement to triage calls from patients discharged on antibiotics. Demonstrate a basic understanding of travel medicine, including counseling patients on recommended vaccinations and administering vaccines during clinic visits as indicated. Maintain accurate documentation in the electronic medical record and communicate effectively with the interdisciplinary care team.
