About The Position

The Nurse Coordinator is a specialized, professional nursing position that supports the Nurse-Family Partnership (NFP) model of home visitation at Any Baby Can. The Nurse Coordinator is the first point of contact for potential clients seeking services at Any Baby Can for Nurse-Family Partnership program services and plays a vital role in ensuring efficient patient flow from referral to enrollment. The Nurse Coordinator may also be required to provide home visits to families enrolled in the program when the assigned Nurse Home Visitor is unavailable and takes on other projects to support professional development and team processes as necessary. The Nurse Coordinator is expected to maintain the highest standards in clinical nursing practice, case management, care coordination, and adherence to the policies, procedures, guidelines, and standards of Nurse-Family Partnership (NFP) and of Any Baby Can.

Requirements

  • Bachelor of Science in Nursing (preferred), Associate Degree in Nursing, plus a minimum of 2 years nursing experience.
  • Current Texas RN license required.
  • Current CPR required.
  • Spanish speaking, reading and writing abilities (required).
  • Excellent written and verbal skills.
  • Must have a strong working knowledge of Microsoft Office programs (Excel, Word, Outlook, etc.).
  • Must have reliable transportation to travel within the assigned regions: if driving own vehicle, must have valid driver’s license and current vehicle insurance.
  • Ability to sit for extended periods of time.
  • Ability to work more than 8 hours per day, including some evenings as needed.
  • Ability to sit, stand, and kneel as necessary to fulfill daily duties assigned.
  • Ability to communicate respectfully, clearly, and effectively.

Nice To Haves

  • Experience in community health, educational or case management setting, with an emphasis on pediatrics, perinatal care, and infant mental health, strongly preferred.
  • Proven ability to communicate and establish interpersonal relationships with low-income and culturally diverse families having multiple needs.
  • Home visiting experience preferred.
  • Demonstrated abilities in problem-solving, initiative, and solution-focused communication required.
  • Demonstrated skills in building self-efficacy, and practicing strength-based, solution-focused nursing with the ability to adapt to change and challenge, required.
  • Possess effective time management and strong organizational skills.
  • Positive attitude, good listening skills, and a strong team player.
  • Open to feedback, positive dialogue with all staff, clients, and community partners.

Responsibilities

  • Use reflective listening to establish trust and build rapport with referred clients and their families.
  • Manage incoming referrals according to established process including maintain organized referral databases and spreadsheets, determine program eligibility, assess referred clients’ health and risk factors according to the NFP model, educate referred clients on NFP model of home visitation, provide resources needed prior to enrollment, schedule and obtain informed consents, and timely coordination and follow up with referral partners.
  • Communicate effectively with clients, families, and other members of the healthcare team (e.g., Nurse Supervisors, Nurse Home Visitors, other program staff at Any Baby Can, OB Care Providers and clinic staff) to ensure coordinated and effective client care.
  • Maintain accurate and detailed documentation of all client interactions, assessments, and interventions in the agency, program and state databases as needed as well as the referral spreadsheet.
  • Adhere to Registered Nurse Scope of Practice, ensure compliance with client privacy regulations (e.g., HIPAA), and follow the nursing process in implementing Nurse-Family Partnership (NFP) model into nursing practice.
  • Maintain all required Nurse-Family Partnership (NFP) education including Dyadic Assessment of Naturalistic Caregiver-Child Experiences (DANCE) training.
  • As needed, performs home visits in accordance with the NFP Model Elements and NFP Visit-to-Visit guidelines and maintains appropriate, thorough, timely, and compliant documentation.
  • Represent the program as needed at Community Advisory Board meetings, stakeholder meetings, program presentations, and other community building activities as necessary to build agency and program awareness and foster strong relationships with community partners.
  • Participate in data review and continuous quality improvement initiatives, including analysis of data trends to inform outreach strategies and support intake to enrollment process improvement efforts.
  • Advocate for individual and community health needs and perform outreach activities within the community as assigned by Supervisor.
  • Support with onboarding and nursing professional development projects as needed to support Nurse Supervisors and the team.
  • Must attend NFP Team Meetings, agency all-staff meetings, and agency outreach committee meetings.
  • May be required to work in the office at discretion of Nurse Supervisor and/or Director of Home Visiting.
