About The Position

This position is eligible for Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Community Living Center (CLC) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. The Nurse (CLC) provides professional nursing care and counseling of acute and chronically ill Veterans based upon the nursing process that includes systematic data gathering, assessment, appropriate nursing judgement, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider.

Responsibilities

  • Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
  • The RN sets priorities, organizes and plans the work to meet deadlines, resolves routine problems, and independently performs work in accordance with unit standards, procedures, and accepted practices
  • Commits to the Veteran/resident, whether an individual, family, group, or community. The CLC RN is responsible for the direct Veteran/resident care delivered in the CLC.
  • Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
  • Responsible for the documented outcomes at the program or service level.
  • The CLC RN collaborates with the Minimum Data Set (MDS) coordinator to ensure accuracy of the Resource Utilization Group (RUG) score.
  • Works with the facility wound team to assess, plan, implement, and evaluate the impact of Quality Assurance, Infection Control, etc., with regard to the care of the resident with wound and ostomy needs.
  • Conducts wound and dressing changes and reviews need for supplies.
  • Assists facility wound nurse with teaching and support of residents with new stomas, and for the evaluation of pressure relief mattresses and/or related equipment for residents in the CLC.
  • Analyzes wound, ostomy, and SCI nursing treatment modalities systematically through data collection, interpretation, and reporting of findings with appropriate recommendations.
  • Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.
  • Fosters a safe and supportive environment conducive to the professional development of health care professionals.
