About The Position

The Registered Nurse works in the Community Living Center at the Coatesville VA Medical Center and is responsible for all elements of the nursing process when providing and/or supervising direct patient care. The incumbent will execute position responsibilities while demonstrating a high level of clinical and interpersonal competency - leadership - broad knowledge base and creative approaches to management of complex systems for the facility.

Requirements

  • English Language Proficiency.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States.
  • Possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Credit for foreign nursing education higher than associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA.

Responsibilities

  • Performs a comprehensive initial assessment of the Veteran's disease process and/or co-morbidity factors, including medical history, medication and allergy history, Veteran's symptoms, pain rating and psychosocial assessment.
  • Develops teaching plans based on assessment of individual needs.
  • Plans and directs nursing care of long term and hospice/palliative care patients admitted to the Community Living Center.
  • Completes all documentation in accordance with regulations and Agency policy and procedure.
  • Coordinates care with other healthcare providers, including spiritual counselor, medical social worker, bereavement services and rehab services as appropriate.
  • Educates the Veteran/family regarding the disease process, self-care techniques, end of life issues and the process for resolution of care and complaint issues.
  • Plans interventions which promote health, self-determination, and dignity at the end of life.
  • Documents observations, assessments, and changes in Veteran's conditions, collaborating with health team members to facilitate positive patient outcomes.
  • Anticipates possible discharge needs and assists patient/significant others and multidisciplinary team in achieving goals for discharge.
  • Addresses/resolves any complaints that fall within scope of authority in a prompt and professional manner.
  • Provides medical care and personal care services to ill and injured Veterans.
  • Provides individualized instruction or directs staff in teaching specific content/skill to adult geriatric and hospice patients/significant others.
  • Identifies physiological, psychological, social, and environmental variable affecting the functional status of the older adult and hospice/palliative care patient.
  • Identifies current coping mechanisms and support systems and their effectiveness.
