About The Position

The University of Iowa Health Care Department of Nursing is seeking a Discharge Nurse Clinician to perform specific tasks to assist members of the Nursing team in providing patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division. The Nurse Clinician for the University of Iowa Stead Family Children’s Hospital Continuity of Care (COC) Program functions as a clinical expert and care coordinator for pediatric patients with special and complex healthcare needs, interfacing with the University of Iowa Children’s Stead Family Hospital faculty and staff from multiple departments/services, patients and families, and referring providers and community agencies/services. The Nurse Clinician works collaboratively with the other COC team members, University of Iowa Stead Family Children’s Hospital faculty/staff, patients and families, and external providers/agencies in coordinating the care of designated patients throughout their inpatient admission(s), discharge to home/community agencies, and return to the University of Iowa Children’s Hospital for follow-up ambulatory visits. The Nurse Clinician provides services coordination, consultation, and resource and referral assistance; is a liaison between the faculty and staff of the University of Iowa Stead Family Children’s Hospital inpatient units and various medical/surgical subspecialty clinics, the regional Child Health Specialty clinics, and numerous interdisciplinary community providers/agencies, as well as patients and their families.

Requirements

  • A Baccalaureate degree in Nursing
  • Minimum of 2 years recent pediatric nursing experience in children's hospital or academic healthcare environment
  • Current license to practice nursing in Iowa
  • Demonstrates skill and efficiency in coordination of care and discharge planning for pediatric patients with complex health care needs (involving multiple departments and external agencies).
  • Demonstrated expertise in specific clinical area of practice
  • Proficiency in computer applications including Microsoft Excel, Word and PowerPoint
  • Excellent written and verbal communication skills
  • Demonstrated excellent organizational skills, critical thinking, and problem-solving skills
  • Demonstrated strong advocacy skills for pediatric patients and families
  • Experience working with an interdisciplinary team
  • Demonstrates willingness to flex work schedule to accommodate patient/family needs (e.g., meet with families during evening/weekends) and participation in community outreach
  • Experience in quality management

Nice To Haves

  • Previous case management experience is desired
  • Knowledge of healthcare financing system, especially Medicaid
  • Experience with development/implementation of EBP standards
  • Experience in ambulatory nursing
  • Certification in case management/pediatric nursing is desired
  • Previous experience with EPIC is desired

Responsibilities

  • Coordinate and/or assist in the planning process for Iowa’s Medical Home and Community-Based Waiver programs.
  • Serve as a resource and liaison for families to regional Child Heath Specialty Clinics and regional medical providers and provide technical assistance and information for children with special healthcare needs being discharged from the University of Iowa Children’s Hospital.
  • Provide follow-up phone calls to families of children with complex special healthcare needs who have been enrolled in the Continuity of Care Program.
  • Be a liaison to families, insurance companies and community providers and assist in securing the in-home community-based services needed for the child to be discharged from the University of Iowa Children’s Hospital.
  • Assist children with special healthcare needs and related services, systems and program development activities as requested.
