Duke Regional Hospital is the second largest of Duke Health's three hospitals and offers a comprehensive range of medical, surgical, and diagnostic services, including orthopedics, weight-loss surgery, women's services, and heart and vascular services.Nurse Clinician - Perianesthsia Administrative Testing (PAT) Clinic Duke University Health System seeks to hire a Nurse Clinician at the PreOp Clinic/Preanesthesia testing unit at Duke Regional Hospital who will embrace our mission of Transforming Lives Transforming Care.   Shift: Monday - Friday, 8am – 4:30pm Salaried position: 40 hours/week   The Nurse Clinician is responsible and accountable for clinical expertise for specific patient populations. The incumbent functions as a clinician, educator, investigator, consultant and leader. Clinical judgments are based on a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems. The Nurse Clinician functions as an integral part of an interdisciplinary team, contributing to excellence in patient care, research, teaching, and in providing leadership to the organization.  

Requirements

  • BSN required
  • 3 years of nursing experience required
  • Current or compact RN licensure in the state of North Carolina required.
  • BLS required.

Responsibilities

  • Coordinates Preoperative/Specialty Appointments
  • Schedules and coordinates all necessary appointments and testing between preoperative visit and surgery
  • Interacts with surgeons’ offices/schedulers and DRH preop schedulers to facilitate patient care
  • Coordinates with DRH preop scheduler to manage clinic appointment slots
  • Follows up with recommendations from appointments and updates patient’s chart and periop medicine providers
  • Follows up on no-shows to preop clinic appointments and other interim appointments, ensuring surgery and periop providers are aware.
  • Determines potential barriers for patients and assists with these (transportation, financial, social, etc.)
  • Monitors and communicates patient updates
  • Monitors patient status board (white board) and determines where patients are in process of surgical readiness
  • Informs periop providers and surgeon if there are potential delays to surgery (as well as medical director and lead periop APP as needed)
  • Communicates estimated time to surgery and/or any changes in perioperative plan to periop providers (as well as medical director and lead periop APP as needed)
  • Communicates changes in treatment plans to periop providers and surgeons (as well as medical director, lead periop APP, and anesthesiology as needed) to determine if surgery needs to be postponed or rescheduled
  • Sends and/or receives notes/messages from specialty appointments via Maestro in-basket and email
  • Disseminates notes to appropriate parties (surgeon, PCP, etc.)
  • Interprets note and determine if optimization plan has changed or will take longer than surgical timeframe
  • Populates a generic note to surgeons, PCPs and other providers regarding any updates on patients (abnormal labs, delay of surgery, need for follow up)
  • Sends note in Epic if able or email/call those providers not in Epic to give patient updates
  • Reviews patient status board (white board) for this week’s pending surgical dates
  • Determines what needs to be done to get patient ready for surgery and help facilitate those processes
  • Ensures all patient are "green" that are going to surgery (fully optimized)
  • Cross-trains and works as needed in associated nursing functions in perioperative areas
  • Communicates with necessary parties to provide for optimal transitions of care, such as from the outpatient to the inpatient setting
  • Collects and presents related data as requested
  • Enters preop phase of care orders for APP/MD to sign off
  • Enters necessary optimization orders otherwise, including orders for preop iron infusion, magnesium infusion, etc.
  • To review potential delays in patients’ surgery
  • To discuss potential barriers within preop clinic/specialty clinics to prevent patient moving forward with surgery
  • To review trends in referral patterns, scheduling, optimization operations
  • To prioritize upcoming work
