About The Position

The Nurse Case Manager coordinates and evaluates health care and social services for participants in the PACE program. Acting as a liaison between the interdisciplinary team (IDT), participants, caregivers, and community agencies, this role ensures seamless care delivery and support. Requirements

  • RN license in the state of the PACE organization; BSN preferred.
  • CPR certification required.
  • 2+ years of experience in geriatric care; 1+ year working with frail or elderly populations.
  • Valid driver's license and insurance.
  • Strong clinical, leadership, and communication skills.

Nice To Haves

  • BSN preferred.

Responsibilities

  • Lead and collaborate with the IDT to develop and implement individualized care plans.
  • Conduct assessments, home visits, and hospital discharge coordination.
  • Maintain accurate documentation and communicate effectively with participants and families.
  • Mentor staff, manage departmental operations, and participate in quality improvement initiatives.
  • Ensure compliance with federal, state, and organizational regulations.
