About The Position

We are seeking an experienced Case Manager RN to support care coordination and utilization management in an acute care hospital setting in Tallahassee, Florida. This role focuses on interdisciplinary collaboration, medical necessity review, discharge planning, and resource management to ensure quality outcomes and efficient patient throughput. The ideal candidate brings strong acute care experience and a working knowledge of regulatory and payer requirements.

Requirements

  • Associate Degree in Nursing or Diploma in Nursing.
  • Active Florida RN license or compact RN license (Florida licensure required within designated timeframe if compact).
  • Minimum of 3 years of recent case management experience in an acute care hospital setting.
  • 3 years of acute care experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED.
  • Home health or insurance case management candidates must have: 3 years total acute care experience, and At least 1 year of acute care experience within the last 5 years.

Nice To Haves

  • Bachelor’s Degree in Nursing.
  • Certification in Case Management, Nursing, or Utilization Review.
  • APRN license acceptable if current and compliant.

Responsibilities

  • Perform comprehensive psychosocial and clinical assessments of assigned patients.
  • Develop, document, and coordinate individualized case management plans of care.
  • Lead interdisciplinary discussions to ensure appropriate level of care and resource utilization.
  • Evaluate admissions and continued stays for medical necessity using approved criteria.
  • Perform utilization management reviews and communicate with third-party payers.
  • Identify barriers to discharge and facilitate timely, appropriate discharge planning.
  • Coordinate referrals to community resources and post-acute services.
  • Track and trend barriers to care; recommend process improvements.
  • Ensure compliance with regulatory standards and internal policies.
  • Promote cost-effective care without compromising quality or patient safety.
