About The Position
We are seeking a Nurse Case Manager to join our team. Salary Range: $58,000 - $65,000 (Salary) We are seeking a Nurse Case Manager to join our team. Your role will be responsible for providing training and consultation on or around medication usage, medical procedure clarification, and the general needs of children in out-of-home care. You will evaluate progress in accessing appropriate behavioral and physical health medical care and other needed services. As a member of the Kansas Foster Care team, you will work with 3 other team members and report to our Senior Manager of Managed Care and Clinical Services.
Requirements
- Our ideal candidate will have at least 1 year of experience in child welfare arena
- Associate’s degree is required
- Active Licensed Practical Nurse (LPN) license in Kansas
- At least 21 years of age and pass background check, physical, and drug screening
- A valid driver’s license in the state you reside in, proof of current vehicle insurance, and reliable transportation
Responsibilities
- Review medical histories, medications, immunization records, and chronic health conditions.
- Coordinate medical, dental, vision, and behavioral health services for youth.
- Provide training / consultation on or around medication usage, medical procedure clarification, the general needs of children in our care, etc.
- Monitor and coordinate follow-up care for youth discharged from hospitals, psychiatric facilities, or PRTFs.
- Ensure proper tracking of psychotropic medication in electronic health record through monthly auditing of the Universal Healthcare form. This includes updating the form when a youth changes placement.
