Position Description: The MyCare Ohio Case Manager is responsible for the assessment and care management of long- term clients ages 60 and older enrolled in the waiver programs. Registered Nurse (RN) licenses are required. At least one year of experience working with individuals with physical or behavioral disabilities in a home health care, medical social work, or behavioral health environment is required.
