Requirements

  • Active Missouri RN license
  • At least 2 years of nursing experience
  • Strong clinical judgment and attention to detail
  • Confidence working independently while staying organized
  • A genuine passion for helping others live well

Responsibilities

  • Complete intake assessments and coordinate care services
  • Develop and manage individualized care plans
  • Monitor health data and adjust care as needed
  • Support and guide caregivers and staff
  • Conduct supervisory visits and provide education to families
  • Address concerns and ensure high-quality service delivery
  • Collaborate with physicians and interdisciplinary teams
