Nursing Case Manager (Part Time) - IDD Services - LPN or RN Office Location: Canton Coverage area: * Brockton/Randolph/Abington/Avon Hybrid Position (flexible schedule) Pay Rate: $31.25/hour (LPN) or $34.60/hour (RN) Daily tasks include but are not limited to coordination of client discharges, intake assessments of new clients, auditing of in home medications and medication administration records, management of new medication orders and training of staff for new diagnoses. The ideal candidate must possess a strong understanding of basic nursing concepts including pharmacology. They must thrive in a fast paced environment and be able to adequately manage multiple tasks within a day. Great communication skills and organizational skills are a must to thrive in this position. They should also have the capability to think outside of the box and a strong desire to learn and grow as a nurse. A general understanding of computers such as email, excel and Microsoft word is beneficial. Knowledge of 6400 regulations are a plus but we will train the right candidate. Candidates must be strongly self-motivated and possess excellent time management skills.
