About The Position
The Nurse Case Manager coordinates and evaluates health care and social services for participants in the PACE program. Acting as a liaison between the interdisciplinary team (IDT), participants, caregivers, and community agencies, this role ensures seamless care delivery and support.
Requirements
- RN license in the state of the PACE organization; BSN preferred.
- CPR certification required.
- 2+ years of experience in geriatric care; 1+ year working with frail or elderly populations.
- Valid driver’s license and insurance.
- Strong clinical, leadership, and communication skills.
Nice To Haves
- BSN preferred.
Responsibilities
- Lead and collaborate with the IDT to develop and implement individualized care plans.
- Conduct assessments, home visits, and hospital discharge coordination.
- Maintain accurate documentation and communicate effectively with participants and families.
- Mentor staff, manage departmental operations, and participate in quality improvement initiatives.
- Ensure compliance with federal, state, and organizational regulations.
