About The Position
The Nurse Case Manager PT - LPN/RN plays a critical role in coordinating and managing patient care to ensure optimal Medication Assisted Treatment outcomes. This position involves assessing patient needs, developing individualized care plans, and collaborating with clients to facilitate effective treatment and recovery. The Nurse Case Manager monitors client progress, advocates for necessary services, and ensures compliance with healthcare regulations and organizational policies. Requirements
- Current and valid Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the United States.
- Minimum of 2 years of clinical nursing and case management experience.
- Strong knowledge of nursing principles, patient care standards, and healthcare regulations.
- Excellent communication and interpersonal skills to effectively interact with clients and healthcare teams.
- Proficiency in electronic health records (EHR) and basic computer applications.
Nice To Haves
- Experience working with substance use disorder and Medication Assisted Treatment clients.
- Ability to work independently with strong organizational and time management skills.
Responsibilities
- Engage clients in the Medication Assisted Treatment program, providing appointment reminders and follow up calls.
- Coordinate communication among MAT program providers and community resources to ensure seamless care delivery.
- Monitor patient progress and adjust care plans as necessary to address changing treatment responses.
- Provide education and counseling to clients regarding Medication Assisted Treatment options.
- Document all case management activities accurately and maintain compliance with regulatory standards and organizational policies.
- Participate in interdisciplinary team meetings to discuss patient care plans and outcomes.
