About The Position

The Nurse Case Manager (NCM) II reports to the Manager of Case Management and Care Transitions. This role functions as a member of the clinical team, coordinating a plan of care to achieve clinical outcomes and promote a safe and timely discharge. The position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system and serves as a clinical expert related to discharge planning, resource utilization, and case management.

Requirements

• Bachelor's degree in Nursing from an accredited school of nursing
• Three years work experience as a Registered Nurse
• Licensure as a registered nurse by the state of South Carolina or compact state
• Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification

Nice To Haves

• Prior leadership experience
• One year of Case Management experience
• Case Management certification
