About The Position

The Nurse Care Navigator LPN works under the supervision of a Registered Nurse (RN) and medical services providers and is responsible for managing patient care needs in the clinic and related to home patients. This includes reviewing all clinical records, obtaining pre-authorizations for medications, managing incoming calls, assisting in the outpatient clinic, and providing education to patients and families on services within Avow Care Services when appropriate. The Nurse Care Navigator LPN is committed to the philosophy, goals and objectives of the Avow Care Services, Palliative Care concept while consistently demonstrating the Avow values and compliance with organization’s services excellence standards.

Requirements

  • Certificate from college or technical school; three to six months related experience and/or training.
  • Current Licensed Practical Nurse (LPN) License for State of Florida
  • Healthcare Provider Cardiopulmonary Resuscitation (CPR)
  • Valid Florida Driver’s License
  • Math skills needed including addition, subtraction, multiplication, division and percentages/fractions with ability to calculate medication doses.
  • Nurse must successfully pass the math test given at time of interview prior to starting orientation and annually.
  • Communication skills of reading, writing, and speaking in English.

Nice To Haves

  • Hospice and or Palliative care experience preferred
  • Certification in Hospice and Palliative Care (CHPLN) preferred; required within 2 years of employment.

Responsibilities

  • Uses the nursing process in collaboration with the RN, APRN, physician, LCSW and chaplain to perform ongoing physical and psychosocial assessments and reassessments to structure age-specific care to individualized patient and family needs.
  • Reviews clinical records on newly and existing referred patients for appropriateness prior to clinic appointment.
  • Demonstrates knowledge in clinical interventions, treatments, pain, and symptom management.
  • Meets with patients and family to explain palliative and/or hospice care and obtains appropriate forms.
  • Demonstrates the ability to perform identified procedures in a clinically competent manner per policy and procedure manual to include interpreting laboratory data and evaluating and documenting patients’ response to intervention/treatments.
  • Demonstrates the ability to assess and interpret patient’s data, in collaboration with the palliative care team, to identify and provide each patient’s age specific needs.
  • Identifies and addresses safety needs to include potential or actual situations and ethical dilemmas in daily practice, communicates as appropriate with all members of the team, and uses appropriate agency resources to address the issues.
  • Checks and responds to all electronic communications in a timely manner including Avow email account throughout the day when scheduled to work.
  • Coordinates care with all internal and external care team members to ensure patient/family needs are met.
  • Attends and participates in clinical education, in-service and nursing staff meetings.
  • Applies working knowledge of state, federal, local and accreditation regulations (CHAP) for the delivery of palliative care services.
  • Other duties as assigned.
