About The Position

The Nurse Care Manager, under the supervision of the Chief of OBGYN (or designee), OBGYN Practice Manager, Service Line Director, Director of Nursing, plays a pivotal role in overseeing the Signature Healthcare (SHC) HEART-BP Program at the OBGYN practice level. This role includes serving as the clinical lead nurse for remote blood pressure monitoring of Signature Medical Group patients at risk for hypertensive disorders of pregnancy. During normal business hours, the Nurse Care Manager will monitor patient data and blood pressure readings flowing into the Babyscripts platform and advise patients on clinical next steps based on evidence-based standards of care and recommendations of OB clinicians. It is the responsibility of the Nurse Care Manager to motivate and educate patients and remove barriers to blood pressure monitoring. Additionally, it is the responsibility of the Nurse Care Manager to coordinate and lead the HEART-BP Grant Implementation Team and SHC HEART-BP Community Advisory Group to meet the requirements of the HEART-BP contract. The Nurse Care Manager ensures that the HEART-BP program operates effectively, improving the monitoring and care of patients at risk for hypertensive disorders of pregnancy, and supporting the broader goals of Signature Healthcare's commitment to quality care. This role is a 32 hr/week hybrid position, requiring travel to the OBGYN practices 2-3 days per week.

Requirements

  • 3 years of OB experience required.
  • LPN or RN

Responsibilities
