The Care Management for At‑Risk Children (CMARC) program is part of our Maternal & Child Health Division, serving children from birth to age five who may be experiencing medical, developmental, behavioral, or environmental challenges. Our multidisciplinary team of Registered Nurses and Social Workers partners directly with families to improve health outcomes and strengthen support systems during the earliest, and most impactful, years of life. We’re looking for a Nurse Care Manager who is passionate about family-centered care and excited to join a mission-driven team dedicated to community health.
