About The Position

East Bay Community Action Program (EBCAP) is seeking a full-time Registered Nurse Care Manager for our Behavioral Health Services Division at our 2 Old County Road location in Barrington, RI. We are seeking highly professional and motivated individuals who enjoy working with a multi-disciplinary team in a community health setting. Key Responsibilities: Provide a wide variety of skilled office-based and community-based nursing services in support of patient treatment plans and documented orders by physicians, and/or advanced practice registered nurses, and/or physician assistants within scope of license and experience. Provide ongoing health assessment (identify health issues, behaviors, needs, barriers, etc.) and all other assessments which are appropriate for the nursing scope of practice. Ensure that all screening tests are up to date with results recorded in the electronic medical record. Triage walk-in patients as well as conduct telephone triage to determine patient need and disposition. Build relationships with medical providers within the organization and in the community, which will provide the nurse with a network of physical health resources. Record and administer medications prescribed by the team physician/psychiatrist; evaluate and chart medication effectiveness, complications, and side effects. Complete Prior Authorization procedures based on patients insurance requirements as needed. Develop and maintain disease or problem-focused patient plans of care. Participate in all daily or organizational team meetings and clinical reviews. Participate in the development, implementation, and utilization of an electronic medical record and electronic prescribing. Complete all necessary documentation in compliance with State and Regulatory Agency policies and procedures, and with published Center standards. Perform other duties as assigned by supervisor.

Requirements

  • Minimum of an Associate's Degree in Nursing or equivalent
  • Licensed as a Registered Nurse (RN) by the RI Department of Health.

Nice To Haves

  • Experience in Psychiatric Nursing preferred.
  • Previous experience in an electronic medical record (EMR) preferred
