Nurse Care Manager - LPN or RN

As a Nurse Care Manager, you’ll be at the heart of client wellness, supporting individuals with chronic conditions and behavioral health needs.
Requirements
- Graduation from an accredited nursing program
- Active, unrestricted Missouri nursing license. RN preferred.
- Valid driver’s license, proof of insurance, and a reliable vehicle
- Familiarity with mental illness and substance use disorders
- Strong communication and customer service skills
- Comfort with electronic health records and intermediate computer proficiency (Microsoft Office and use of email)
Nice To Haves
- 2+ years of nursing experience in a mental health setting
Responsibilities
- Collaborate with an interdisciplinary team to develop client-centered wellness plans
- Conduct health and metabolic screenings (quarterly or annually)
- Provide outreach calls to engage and support clients
- Lead health and wellness education sessions and group activities
- Monitor hospital admissions/discharges and coordinate medication reconciliation
- Partner with healthcare providers to ensure continuity of care
- Track and assess performance measures related to physical health
- Occasionally (up to 25%) meet clients in the community or in their homes
