About The Position

We are a fast-paced, growing practice who is a leader in the Nephrology space, and we are looking for a Nurse Care Manager to join our team. The key functions of this role include addressing each patient’s specialized needs based on their individual conditions, healthcare needs, goals, and wishes. This position will collaborate with a team of physicians, Advanced Practice Providers (APPs), and Interdisciplinary Team (IDT) members along with our partner Evergreen Nephrology to manage assigned patients and address each patient’s specialized needs. Your job duties will include taking full ownership of patient care management with an additional focus on helping patients with kidney disease navigate the kidney care continuum and manage their chronic conditions post hospitalization. There will also be a focus in helping patients transition from Post Acute Care to home. You will perform assessments to identify needs based on individual values, goals, and preferences, which informs your development of comprehensive care plans for each member. Requirements

  • Graduation from of an accredited school of nursing
  • Possession of an IL state registered nurse license
  • Minimum of three years of experience in telephonic care management and post-acute care

Nice To Haves

  • Strong understanding of skilled nursing facility operations and regulations strongly preferred
  • Knowledge of value-based care models and strategies to reduce readmissions
