JOB SUMMARY Coordinates, negotiates, communicates, and manages the care of assigned patient population along a continuum to facilitate achievement of optimal patient outcomes within appropriate LOS and resource utilization parameters.
Requirements
- Graduate of a school of nursing.
- Two (2) to Three (3) years recent clinical nursing experience in an acute care setting.
- Current unrestricted License in good standing to practice as a Registered Nurse in the State of Connecticut.
- Current Basic Life Support (BLS) certification.
- Ability to obtain and maintain department specific certifications.
Nice To Haves
- BSN or MSN preferred.
- Certified Nurse Care Manager preferred.
