About The Position

As a Nurse Care Manager LPN for Swope Health Services, you will be a part of a team you will be part of a team of caregivers to provide care for the people who need it most. Our Nurse Care Manager manages a wide array of physical health needs, and behavioral health needs. Requirements

  • Current, unrestricted license as LPN in the State of Missouri and Kansas
  • License to start, willing to hire LPN without Kansas license if eligible to obtain Kansas license within the first 90 days of employment.
  • 2-5 years of experience in an outpatient clinical setting or previous experience in care coordination role

Nice To Haves

  • BSN preferred
  • pediatric experience preferred
