About The Position
Nebraska Medicine is seeking a Nursing Care Coordinator to join the Gender Care team, this role is located at the Specialty Services Pavilion in Omaha, NE. The Nursing Care Coordinator will be responsible for patient assessment and planning, and overall care coordination through a multidisciplinary team collaboration. As well as providing patient education, serving as a liaison between patients and healthcare professionals, and regularly reviewing and adjusting patient care plans based on progress and feedback.
